ACCEPTED INSURANCE PLANS

We are In-Network with

Blue Cross PPO of IL Plans.

We do not accept any other insurance at this time due to the hassle for our clinical staff to get reimbursed by many of the modern insurance companies. If you do choose to use your insurance be aware that your Deductible may not be met and this would mean you will have to come out of pocket before your Deductible is met. You are responsible for this as this is different with every BCBS Plan.

All patients are seen every 1-2 months for Mental Health Surveillance and Medication Management.

We do accept patients as out of pocket pay.

New Patient Consults: $250 

Follow-Up Consults: $150 

​

All Consults are seen by a Board Certified Physician.
